As for cladding the building, from what I've read this far the original specified material was fine but somewhere between appetite and arsehole the spec was changed in order to reduce cost and an inappropriate cladding was used for the application and therein lies the problem. The process of managing change was self evidently flawed but attributing blame, therein is the problem because it is probably a failed or even non existent formal change process that was in place. It's going to be interesting to see who eventually carries the can - if anyone can be nailed down.
