In 2003, an NHS colleague first raised concerns, eventually, suspended eight years later.

https://www.bbc.co.uk/news/health-51380454Add to that the power and status of a surgeon in the medical world and, in the words of the report, Paterson was "hiding in plain sight". It's clearly impossible to eliminate the activities of determined criminals in any profession.
First thing that needs to change, obviously.
