I don't know why charging people for missed appointments wasn't implemented a long time ago, some selfish people know there is no punishment for missed appointments so don't bother cancelling, they just don't turn up.

Whilst they may be good ideas some of those things will never work. locally our gp surgery opened Saturdays, sadly they forgot to tell the patients and didn't offer any appointments so they deemed it a failure and said no one wanted it.
Many hospital departments send out letters stating you will attend at a time and place, then get all upset when the patient can't get there.
The NHS needs to be more flexible, some seem very adverse to any changes.
Waste is rife too.

So good ideas but need a lot of changes to make them work.
