Most people don't. In fact it's usually not severe cases that cause long covid. They think it's partly down to the virus not being completely eliminated from the body because there wasn't a severe illness, perhaps no fever etc to kill it off. Like a low level infection when you have say a localised problem or something like shingles or herpes that is semi dormant but flares up and incapacitates.

They are still learning about the virus of course so we don't know the long term effects of infection. e.g. I understand that once upon a time rheumatic fever caused long term effects which manifested decades later in heart problems.
