I think the main factor is that he was dangerous, not the fact that he had a mental illness. He should never have been in a public place whether escorted or not, he should be in a secure hospital. The so called carers are just as much to blame for thinking they could control this nut job in public.

I visited a secure unit once and was not impressed by the staff .
They are not the brightest of people .
Most are untrained .
It’s unpleasant low paid work who would want to look after a dangerous violent teenager ?

This man should not have been in a public place. Clearly a mistake. No doubt an enquiry will give details, was he on a rehabilitation programme, and the usual lessons will be learned.Those responsible for his presence at Tate Gallery, will not be held to account.

In any event, I can well believe what you say about 'carers' and the whole system of how these 'mentally challenged' people are dealt with. I think the same goes for social workers, they all seem to be of a type.
The big problem, I think, is the power and influence possessed by our 'apologists', who have also obtained five-star standards for prisoners! It is since they took over from the more hard line system of years gone by that much of these sort of problems have become more prevalent.
