The only thing I am sure about is that I am booked to go to the same place on 27th April to get my second jab.
This was new to all of us so I am sure that a lot of us don't remember everything Art!

The nurse was waiting for me and l finished filling it in after my injection.
Even if you tortured me, l wouldn’t be able to tell you, specifically what the form was like!
I could tell you, it was snowing on the day and the woman sitting opposite me was born in the same year as me and looked about 100.
The nurse had short hair and glasses and a tattoo on her arm. The seat l sat in was red! All true!
I booked the appointment via a text message from my doctor’s surgery. I just chose a time on the day they offered me at a local hospital. No other details were asked for.

So didn’t others on this thread have to fill a form in before their vaccination?
l said l was concerned by the fact l suffered allergies but as l mentioned earlier, it didn’t seem to be a big deal unless you’d ever suffered anaphylactic shock and had one of those pens.
